Please note that ALL information is required for your application to be properly processed. Any missing or incomplete information will result in a delay or deletion of your registration form.

858AIRSOFT.COM EVENT REGISTRATION FORM

* All information below is REQUIRED including addresses and contact phone numbers. The information requested will be used to confirm your attendance of the selected 858 Airsoft event. Failure to provide the required information below will result in the possible delay or deletion of your registration form. In the event that a registrant has been deleted from the registration process, he or she will NOT be notified and it is the responsibility of the participant to resubmit a new registration form.

* There are NO REFUNDS OF EVENT FEES if in fact the event is scheduled to take place. In the event that a game is cancelled or postponed due to weather or similar circumstances beyond our control, all registration fees will be held and applied to either the rescheduled game or to the next event hosted by 858airsoft.com.

* All participants regardless of frequency of game attendance must fill out an insurance waiver for each person attending. Insurance waivers are NOT transferable and are NOT considered valid except for the event in which it has been filled out for. Participants under the age of 18 years old (except AL, NE, WY (19) and MS (21)) must have the proper insurance waiver filled out and signed by his or her parent or legal guardian.

* Once a registrant has checked in with the game control staff at any selected 858airsoft.com event, that participant has taken possession of the services offered by 858airsoft.com regardless of his or her actual completion or participation of the event. If the participant so chooses or has been asked to leave the event early for ANY reason, he or she acknowledges that 858airsoft.com has fulfilled their financial responsibility and will at no time request or require any type of refund or reimbursement of any kind.

* By completing this Event Registration Form, the person named on this registration form agrees to pay 858airsoft.com the required event registration fee as per set forth for the event. In addition this registrant understands that any dispute or other contesting of the registration fee is considered fraud.

PLEASE FILL OUT ONE (1) REGISTRATION FORM PER PLAYER.
DO NOT 'COMBINE' MULTIPLE PLAYERS ON THIS REGISTRATION FORM.
Each player MUST have his/her OWN registration form so that we can properly fill out the players rosters and compare the registration forms. Even if you combined payments (which is okay) we STILL need a SEPARATE registration form PER PLAYER.

** Your "PAYMENT CONFIRMATION NUMBER" is the transaction or receipt number that you were given after you had completed your payment. This number is important to us, as this is the ONLY way that we will be able to confirm your payment to us in the event of a data failure beyond our control. Please make sure that you keep a verifiable record of your payment so that in the event we do not have your payment on file, you may show us proof on request. Please note, we have rarely had any problems with finding participants payments and the ability to cross reference the payment confirmation number to the player.
